Humana Inc, a leading U.S. healthcare company, has announced new strategic partnerships aimed at improving heart health outcomes for eligible Medicare Advantage members through expanded value-based care programs. By collaborating with Karoo Health, US Heart and Vascular, and Chamber Cardio – while continuing its partnership with CVAUSA’s Novocardia Care Solutions division – Humana is strengthening its comprehensive cardiac care strategy to provide more coordinated, proactive, and patient-centered healthcare services.

This expansion reflects a growing focus across the healthcare industry on value-based care models that prioritize preventive care, improved patient outcomes, and reduced hospitalizations. Through these partnerships, eligible Humana Individual and Group Medicare Advantage members will gain access to enhanced cardiac care services delivered through participating cardiologists and integrated healthcare teams.

The initiative is designed to support patients living with cardiovascular conditions by providing continuous monitoring, coordinated treatment plans, and improved access to medical professionals. Value-based care teams will focus on proactive and evidence-based care strategies aimed at preventing complications, reducing unnecessary emergency visits, and improving long-term patient health outcomes.

Caraline Coats, Senior Vice President of Clinical Strategy at Humana, highlighted the importance of integrated care for heart patients. “Managing a heart condition can feel overwhelming. Our members need integrated, coordinated, evidence-based care that meets them where they are,” she said. Coats also emphasized that expanding access to whole-person healthcare remains central to Humana’s strategy for improving population health outcomes.

As part of the program, participating members will benefit from several key features designed to enhance ongoing cardiac care management. These include 24/7 access to coordinated care teams through a dedicated hotline, enabling members to receive support and guidance whenever health concerns arise. Remote monitoring tools will also allow healthcare teams to track heart rate, rhythm, blood pressure, and other key health indicators, helping clinicians detect potential complications before they escalate into serious medical events.

In addition, integrated care teams – including care managers and cardiologists – will collaborate to develop personalized treatment plans tailored to each patient’s specific needs. This coordinated approach ensures alignment across medication management, diagnostic testing, and lifestyle recommendations such as nutrition and physical activity.

Humana’s internal research suggests that value-based care models lead to improved quality of care and stronger adherence to evidence-based treatment protocols among cardiac patients. By incentivizing collaboration between healthcare providers, these models encourage more holistic care delivery while supporting better long-term outcomes.

Through these new partnerships, Humana Medicare Advantage members will gain access to cardiac care services across multiple states. Novocardia Care Solutions services will be available in Florida, Georgia, South Carolina, Illinois, North Carolina, and Iowa. Karoo Health will provide services in Arizona, Illinois, Alabama, Mississippi, and Louisiana. Chamber Cardio will serve members in Maryland, Georgia, South Carolina, North Carolina, Virginia, Washington, DC, and Texas, while US Heart and Vascular will provide services in Alabama, Arizona, Louisiana, Texas, and Kansas.

By expanding these value-based cardiac care partnerships, Humana continues to strengthen its commitment to delivering coordinated healthcare solutions that support heart health, improve patient quality of life, and reduce preventable hospitalizations among Medicare Advantage members.
