Manifest MedEx, California’s largest nonprofit health information organization, has partnered with NinePatch, a leading SaaS platform for Whole Person Health and Referral Information Exchange, to introduce a statewide social health information exchange platform. This new platform will integrate social determinants of health data, care assessments, program enrollment, consent management, and closed-loop referrals with more than 50 million longitudinal health records on Manifest MedEx’s network. Powered by NinePatch technology, the platform will be available to over 140 hospitals, 19 health plans, 12 county health agencies, and 2,700 providers across California.

Healthcare providers and community-based organizations in the MX network will be able to securely share patients’ health histories, social needs, and social service information according to permitted access. By combining these data into a single unified view, the platform helps providers, social service organizations, and Medi-Cal plans deliver Enhanced Care Management and Community Supports under CalAIM more efficiently.

Erica Galvez, CEO of Manifest MedEx, said the partnership empowers communities by giving participants the tools and comprehensive data needed to streamline care coordination, reduce duplicated services, and improve overall community health and wellness. NinePatch, originally launched through award-winning community initiatives in Colorado, focuses on connecting health and social care services to promote whole person care. Its patented OneView tool simplifies complex data into clear care pathways and supports needs identification, case management, referrals, program access, reporting, billing, and outcome measurement.

The first implementation of MX’s SHIE platform will take place in a large county in California’s Central Valley, advancing local community data exchange strategies and supporting public health and CalAIM objectives. County departments and contracted CBOs will be able to track, report, and share social data securely while maintaining strict access controls. The platform allows role-based permissions to coordinate care across multiple departments and service organizations.

Manifest MedEx plans to offer its SHIE platform to California county health agencies starting in 2026. MX is a designated qualified health information organization under California’s Data Exchange Framework and one of the first HIOs in the nation to participate in TEFCA, the Trusted Exchange Framework and Common Agreement. Alongside NinePatch, MX is also partnering with Findhelp, a social care referral platform, to ensure secure and appropriate access to social determinants of health data for all network participants.
