PocketRN, a top provider of virtual nursing services, has teamed up with Comfort Keepers, a well-known in-home care company, to start a national partnership. This collaboration is part of the Centers for Medicare & Medicaid Services’ (CMS) new dementia care program called Guiding an Improved Dementia Experience (GUIDE). The program is designed to improve the quality of life for people living with dementia and give support to their families and caregivers.

PocketRN has been selected as one of nearly 400 organizations across the country to take part in the initiative. They will manage Dementia Care Programs (DCPs) that offer reliable and accessible care to families everywhere. Comfort Keepers will help by providing in-home respite care and extra support through their network of trained caregivers, many of whom specialize in dementia and Alzheimer’s care.

The CMS GUIDE Model, which began on July 1, 2024, includes detailed assessments, 24/7 access to care teams, coordinated services, and options for caregiver respite. Through this partnership, PocketRN and Comfort Keepers are committed to increasing access to education, specialized dementia support, and personalized in-home care. Together, they are working to create a care solution that is both scalable and sustainable, offering families the help they need when they need it most.
