HealthSnap, a top company in virtual care management, shared that University Hospitals (UH), a major academic medical system in the country, has launched a new Remote Patient Monitoring (RPM) program using HealthSnap’s technology. The program started in July 2025 and focuses on helping patients with difficult-to-control hypertension. Hypertension affects over 30 percent of adults in Ohio and leads to thousands of unnecessary hospital stays each year. This program provides ongoing support to help patients better manage their condition, which in turn helps cut down on hospital visits and improve overall health results.

University Hospitals has many hospitals and outpatient centers throughout Northern Ohio. It is known for its focus on clinical innovation, academic quality, and research. Once the RPM program is fully launched, it will cover almost 100 practice sites under the UH Primary Care Institute and assist more than 400 healthcare providers.

“University Hospitals is committed to exploring new ways to deliver care,” said Dr. Anthony Muni, Chief Medical Officer of Utilization Management & Clinical Documentation Integrity. “Working with HealthSnap allows us to better support patients with uncontrolled hypertension between visits, keeping them connected, following their treatment plans, and on a path to better health.”

The program aims to reduce avoidable hospital stays, improve blood pressure control, and allow earlier clinical actions by giving care teams continuous information on patients’ vital signs and symptoms. HealthSnap’s platform works well with UH’s current workflows and electronic health records, making it easier for both healthcare professionals and patients to use.

“University Hospitals is setting an example for modern chronic care,” said Samson Magid, CEO of HealthSnap. “We are proud to support their exciting vision with scalable RPM solutions that deliver real results without adding extra work for healthcare providers.”

This partnership marks a big step in using technology to improve the management of chronic diseases and ease the pressure on healthcare facilities by keeping patients and providers connected in real time.
