Santa Clara Family Health Plan (SCFHP) has collaborated with Quality Health Partners (QHP) to offer our Medicare and Medi-Cal members another important touchpoint with a healthcare professional. QHP provides health assessments and well-visits to members via telehealth or in-home, gives them one-on-one attention and support, and relays information to the member’s primary care provider. By entering this partnership, SCFHP hopes to close gaps for members in accessing their care.

This partnership includes health and wellness visits such as the Medicare Annual Wellness Visit, child well-visits, postpartum care visits, and other important health check-ups. These one-on-one visits may be virtual or in-person check-ins with a licensed health care professional. Preventive care visits are essential for members to learn about the health issues they face and for providers to understand how to best serve the member’s needs. By giving members control of their health, SCFHP and QHP believe members will achieve better health outcomes.

As part of this working relationship, SCFHP and QHP recently hosted a pop-up event at the SCFHP Blanca Alvarado Community Resource Center (CRC). Approximately 45 visits were completed in one day by QHP staff at the CRC in a friendly, community setting. During each visit, members were welcomed by SCFHP staff and met with a QHP health care professional, and left with a better understanding of their health and how to navigate the healthcare system to access additional services.

“The health plan is very good. If we have any questions, there’s always a nice person who answers the phone,” said a member who attended the event. “Sometimes there are things that you don’t know, so there’s someone there who can help you understand [the member journey] better.”

The partnership between SCFHP and QHP will hopefully increase access to care by breaking down barriers such as transportation, mobility issues, and long wait times. Ultimately, this approach will enhance continuity of care, encourage regular check-ups, and improve overall health outcomes. SCFHP looks forward to continuing its relationship with QHP as it strives toward Healthcare.
