Chapters Health System, the nation’s leading chronic illness innovator and largest nonprofit hospice network, announced the finalization of an affiliation agreement with Hospice East Bay (Pleasant Hill, CA) as part of the Chapters Health West division. The Chapters Health West division also includes Hospice of Santa Cruz County (Santa Cruz, CA), Nathan Adelson Hospice (Las Vegas, NV) and Willamette Vital Health (Salem, OR), all of whom finalized their affiliations earlier this year.

For nearly 50 years, Hospice East Bay has provided exceptional care, support and bereavement services to patients and families facing chronic illnesses. In 2024, Hospice East Bay cared for more than 1,300 patients in the communities it serves and provided more $308,000 in charitable and uncompensated care to those in need.
