Cityblock Health and Meridian Health Plan of Illinois, Inc. announced a new partnership to provide comprehensive community-based care to 10,000 Medicaid members across 17 counties in the greater Springfield and St. Louis Metro East areas, covering both urban and rural communities.

Meridian, a leading managed care organization in Illinois and part of Centene Corporation, brings an established provider network to the collaboration. Cityblock, a value-based healthcare provider serving Medicaid and dually eligible members, has been operating in Illinois since 2024. This marks the fourth regional partnership between Cityblock and Centene, building on existing collaborations in New York, Ohio, and Florida.
The partnership will enhance care coordination for Medicaid members by combining Meridian’s primary care network with Cityblock’s wrap-around services. Cityblock’s model provides 24-hour access to a wide range of services including primary and urgent care, behavioral health, social support, clinical pharmacy, and care management, ensuring a seamless and integrated care experience.

Cristal Gary, President and CEO of Meridian Plan, said the collaboration strengthens the organization’s ability to deliver value-based care to Medicaid members. She stated that Cityblock’s innovative approach is specifically designed to address the complex health needs of high-risk populations and that the partnership aims to improve access, equity, and outcomes for members.
Mike Roaldi, President of Cityblock Health, emphasized that working with Meridian allows the company to expand its reach within Illinois and better serve the state’s Medicaid population. He noted that nearly one in four residents of Illinois are enrolled in Medicaid and highlighted the importance of scaling access to high-quality care for this complex patient group. Roaldi added that the partnership leverages Cityblock’s data-driven approach, multidisciplinary clinical expertise, and member engagement strategies to enhance both quality and patient experience.
Through this collaboration, Cityblock and Meridian aim to create a comprehensive, integrated care system that addresses the physical, behavioral, and social needs of Medicaid members while strengthening value-based care delivery and supporting long-term health outcomes across the Illinois communities they serve.
