Cityblock Health and Mercy Health announced a strategic partnership to serve Medicaid patients assigned to Mercy Health primary care providers in the greater Cincinnati and Toledo regions. Under this collaboration, Cityblock will extend care services that complement Mercy Health’s existing primary-care network, delivering coordinated support across primary and urgent care extension, behavioral health, and social care. The services will be available 24/7 through in-person, virtual, and home-based delivery modes.

Mercy Health, operating locally as part of the Bon Secours Mercy Health system—one of the largest non-profit Catholic health systems in the U.S.—and Cityblock, a value-based provider with deep experience serving the Medicaid population, say they share a mission of providing compassionate, high-quality care while improving outcomes for underserved communities in Ohio. By integrating their capabilities, they aim to expand access to care, engage patients who are often hard to reach, address social determinants of health such as food and housing insecurity, and reduce avoidable emergency department visits and hospitalizations.

Dr. John Luellen, State President – Ohio, Mercy Health, noted that this collaboration reinforces Mercy Health’s commitment to delivering patient-centred healthcare and allows them to better meet the evolving needs of the communities they serve. He explained that working with Cityblock’s proven model enables Mercy Health to expand access to compassionate, high-quality care for Medicaid patients by meeting them where they are and ensuring their full spectrum of needs is addressed.

Mike Roaldi, President at Cityblock Health, commented that Mercy Health’s longstanding history of clinical excellence in Ohio sets a strong foundation, and Cityblock is honoured to build on that work by integrating its wrap-around care model to support Medicaid patients beyond the doctor’s office and into their homes and communities.

Cityblock’s care model is designed specifically to address the complex challenges facing Medicaid populations, many of whom face disproportionately high risks of poor health outcomes. The model uses purpose-built technology and data insights, alongside a best-in-class engagement strategy, to deliver integrated care. Having operated in Ohio since 2021 and currently serving more than 30,000 members in the state—including through community clinics in Columbus and Cincinnati—this new collaboration marks an expansion of Cityblock’s offerings in Northwest Ohio.
