Lightbeam Health Solutions, a leader in AI-enabled population health management, announced that Integrity Health, serving employer populations in New Jersey and Pennsylvania, has achieved notable improvements in patient outcomes and cost reduction using Lightbeam’s population health platform. Analysis of data from Integrity Health’s Partnership Health Centers (PHCs) shows that patients treated within the PHC network experience better outcomes and lower costs compared to those seen outside the network, demonstrating the benefits of coordinated, patient-centered care.

Patients cared for through PHCs had average inpatient costs of $21,601 per claim, compared with $25,633 for patients treated outside the network, representing a 16 percent reduction in per-claim costs over a 12-month period. PHC patients also had shorter hospital stays, averaging three days compared to four, and experienced six percent fewer 30-day readmissions than those outside the network. Given that avoidable readmissions can cost employers more than $32,000 per claim, these reductions translate into substantial savings and demonstrate the effectiveness of proactive, coordinated care.
Integrity Health’s Care Coordination program further highlights the impact of targeted management. Patients engaged with PHC RN Care Coordinators saw their average monthly costs decrease from $2,001 to $1,667 per member per month. Inpatient admissions and readmission rates also declined, reflecting the strength of Integrity’s clinical model and the value of Lightbeam’s analytics and decision-support tools in managing costs while maintaining high-quality care.

Patient engagement within PHCs is strong, with 92 percent of patients keeping office visits between July 2024 and June 2025, compared to 88 percent for patients treated outside PHCs. High engagement supports early interventions, better chronic disease management, and reduced downstream costs.

A man in his late 40s who had suffered a stroke and was not connected to primary care was identified through Lightbeam analytics. Care coordination enabled him to regain health stability, manage chronic conditions, return to work as a woodworking teacher, and significantly reduce his healthcare costs and risks. Lightbeam’s insights were critical in identifying and engaging him, demonstrating the platform’s real-world value in improving patient care and controlling costs.
