HabitNu, a digital health company recognized by the CDC for its work in lifestyle-based prevention of diabetes, obesity, and other chronic conditions, has been chosen by the Centers for Medicare & Medicaid Services (CMS) to be part of the National Health Tech Ecosystem. This initiative is part of CMS’s efforts to modernize healthcare delivery by improving access to data, promoting interoperability, and making digital health tools more personalized and accessible to patients.

Through this collaboration, HabitNu joins a group of selected organizations focused on building secure, real-time data links between CMS systems and the apps or platforms that patients and providers rely on every day. The National Health Tech Ecosystem, led by CMS Administrator Dr. Mehmet Oz, is working to create a connected network of electronic health records, mobile apps, and digital tools that improve healthcare outcomes while reducing the burden on healthcare providers. This approach is intended to give patients more control over their health information and enhance the quality of care they receive.
HabitNu has made several commitments to support the goals of the ecosystem. These include using secure APIs and identity systems to connect with CMS-aligned networks, offering behavior-based coaching and personalized health nudges, and automating the patient onboarding process by pulling verified data directly from CMS records. The company will also ensure secure, consent-based data sharing that meets HIPAA standards and will report key outcome measures such as improvements in weight, HbA1c levels, medication adherence, and engagement.

These commitments are especially important for Medicare and Medicaid populations, who are often affected most by chronic diseases like diabetes and high blood pressure. HabitNu’s platform offers mobile-first tools that track behavior, provide culturally tailored content, and offer one-on-one coaching. Already used by public health agencies, Medicaid and Medicare plans, and employers, the platform will now be able to act on real-time data from CMS systems, allowing timely interventions when clinical risks such as missed medications or new diagnoses occur.
“This isn’t just a pledge – it’s a shift in how prevention actually happens,” said Dr. Sindhu Rajan, CEO of HabitNu. “Behavior change doesn’t start in the clinic. It starts with the habits people build in their homes, jobs, and communities. CMS is laying the technology infrastructure for real-time data access. Our job is to make that data actionable, empathetic, and personal. This ecosystem allows us to meet people in the moment, with the right nudge, the right coaching, and the right context, before problems become crises.”
